After the Loading Dose: Vitamin D3 Maintenance and What to Monitor

Most people find the Vitamin D3 Anti-Inflammatory Regimen through the loading dose. The number is memorable. The schedule is short. The hope is immediate relief.

What often gets less attention is the next phase: maintenance. That is where the regimen stops being a short course and becomes a monitored, long-term plan. If loading is how many people start, maintenance is how many people stay well.

This article explains what the current guides say after loading, which blood tests still matter, and how to think about dose changes without treating any single IU number as a guarantee.

What the guides say after loading

The current Quick Start Guide describes a total Vitamin D3 loading dose of 600,000 IU, usually spread over 12 days at 50,000 IU/day or 6 days at 100,000 IU/day. After either schedule, it drops back to an initial maintenance dose of 10,000 IU/day, or 50,000 IU/week with the Bio-Tech D3-50 preparation.1

That drop matters. Loading is temporary. Maintenance is the dose intended to keep serum 25(OH)D in a useful range while calcium safety stays under review. The older Full Reference Guide for neurologists used the same idea: load, then maintain around 10,000 IU/day and adjust with labs.2

What the post-loading labs are looking for

The Quick Start Guide asks for repeat assays of 25(OH)D3, calcium and PTH either about 15 days after finishing the 100,000 IU/day schedule, or about 30 days after starting the 50,000 IU/day schedule.1

The practical targets described in the guide are:

  • a rise in 25(OH)D3 of about 55 to 60 ng/mL above baseline
  • serum calcium within its normal reference range
  • PTH lower than baseline

NIH Office of Dietary Supplements material is useful here as a safety backdrop: vitamin D status is assessed with serum 25-hydroxyvitamin D, and toxicity concerns are mainly about high calcium and related complications rather than the IU label alone.3

Maintenance is not “set and forget”

After loading, many people assume the hard part is over. In practice, maintenance is where the regimen either becomes sustainable or slowly drifts.

A useful way to think about it:

Question Why it matters
What is my current 25(OH)D? Shows exposure/status after loading and during maintenance.
Is calcium still normal? The main near-term safety marker for Vitamin D excess.
What happened to PTH? Helps show how calcium-control hormones are responding.
How are attacks changing? Symptom logs make dose decisions less guesswork.
Am I still taking cofactors? Magnesium, omega-3, vitamin K2 and the rest are part of the regimen, not optional extras in the guides.

The Quick Start Guide treats maintenance as ongoing. Dose may move up or down with clinician review. Cofactors continue daily. Hydration remains important, especially if the maintenance dose sits above 10,000 IU/day.1

Why people lose ground after a good start

From years of watching patient discussions around this protocol, the post-loading problems I see most often are practical rather than mysterious:

  1. Stopping too early. A few good weeks are not the same as a settled pattern.
  2. Skipping cofactors. Magnesium in particular is easy to drop when bowels get annoyed, then hard to restart consistently.
  3. No follow-up labs. Without 25(OH)D, calcium and PTH, dose changes become guesswork.
  4. Confusing loading with maintenance. People either stay on a loading intensity too long, or drop so far that serum levels slide.
  5. Ignoring other inflammation drivers. The guides themselves point to situations where allergies or other inflammatory load may blunt response.

None of those points prove the regimen works for everyone. They do explain why a strong first response can fade if the maintenance phase is treated casually.

How to talk to your doctor about maintenance

Bring the guide, your headache log, and your lab printouts. Then ask concrete questions:

  • Are my calcium and kidney markers acceptable for continuing this dose?
  • Is my 25(OH)D rising as expected after loading?
  • Should maintenance stay at 10,000 IU/day, move to weekly 50,000 IU, or change based on these results?
  • How often should we recheck 25(OH)D, calcium and PTH?
  • Do any of my medications or conditions change the safety picture?

Where acute care still belongs

Maintenance of the Vitamin D3 Regimen is a preventive discussion. It does not replace acute cluster headache care. European Academy of Neurology guidance still strongly recommends high-flow oxygen and subcutaneous sumatriptan for attacks, with verapamil among the preventive options clinicians consider.4

That distinction matters. A nutritional anti-inflammatory regimen and an emergency abortive plan answer different questions. Many patients use both under medical supervision.

The takeaway

Loading gets people through the door. Maintenance keeps the door from swinging shut.

If you have finished a loading schedule, the next useful questions are not “was 600,000 IU a big number?” They are: what did 25(OH)D do, is calcium safe, what happened to PTH, are cofactors still in place, and what maintenance dose does your clinician support from those results?

That is the quieter half of the regimen. It is also the half that decides whether a promising start becomes a durable plan.
